Summary: Summary: A single active VIMS fan with centralized pressurize monitoring
panel is in a small mechanical room accessed from door on eastern side of building
adjacent to Chipotle restaurant. No VIMS piping labeling or system alarm was present.
The installation of both was detailed in approved VIMS plan. Spoke with consultant
(Patrick Lowery, Summit) via phone during inspection, who confirmed that
labeling/system alarm were not present. Formal notification to developer pending.

The current risk to the public is unknown at the time of the site inspection. Due to
historically low vacuum influence readings under the Buffalo Wild Wings restaurant,
Summit Engineering install a soil gas point in the vicinity of the restaurant to determine if
soil gas VI in this area of the building is a concern. The soil gas sampling point was installed
in August 2020, though a report has not yet been submitted to DEQ.
During the site visit, the two pressure monitoring points under Buffalo Wild Wings
indicated no vacuum influence initially (<0.000“ WC), which is consistent with historically
low vacuum readings observed under this portion of the building. A smoke pen indicated
that at least one 2” electrical conduit was under some amount of vacuum influence. Any
potential pathways for air infiltration from sub-slab should be sealed to eliminate VI
pathways, with the potential benefit of increasing sub-slab vacuum influence.

Photo 1: light gray door to mechanical room containing VIMS components.
Obtained keys to mechanical room from manager at Buffalo Wild Wings. During site visit,
contacted Summit PM to discuss findings. Requested that VIMS pipe labeling be
installed, installation of valves on ends of pressure monitoring tubing, and that additional
sealing within conduit containing polyethylene pressure monitoring tubing be performed
as smoke pen indicated at least one leak in conduit annulus. Confirmed with PM that
there was no VIMS alarm on system although alarm system was in the approved VIMS
plan.

Photo 4: Vacuum measurements taken during Site Inspection. The highest vacuum of
0.083"WC was detected at Chipotle Front monitoring point, although no vacuum (<0.000" WC)
was indicated at both the Buffalo Wild Wings front and back monitoring points on arrival.

There were no valves on vacuum monitoring tubes to prevent VI into mechanical room and
potentially into other occupied spaces in the event of a VIMS fan failure.
