The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
ADDITIONAL COMMENTS:
1. The purpose of this site visit was to inspect Phase 4 Cell 4-B prior to waste being accepted in this new cell.
2. All photos included in this report were taken by Susan Heim during the inspection on June 14, 2021.
Field Inspection:
3. Prior to inspecting Cell 4-B, a few areas of the active landfill were observed as a follow-up to the previous
inspections on February 24 and April 23, 2021 when several concerns about landfill cover, windblown litter,
and scrap tire accumulations were expressed.
4. Scrap tires were being loaded into a trailer at the time of this inspection. An accumulation of several thousand
tires were observed on the concrete pad in the collection area, along with tarps that partially covered the back
side of the tire pile. Mr. Shelton stated that the county was still experiencing problems with having adequate
trailer space and tire removal from their tire disposal contractor, New River Tire. He confirmed that facility
staff ensure the tire pile is covered daily, as required.
5. Vegetation had become established on the closed area of the active landfill since the previous inspection. The
side slopes were observed to be stable, with no evidence of erosion noted.
6. When partial closure of Phases 1, 2 and 3 took place, the contractor had removed many of the edge -of -waste
markers as they disturbed the slopes to regrade and apply final cover. During this inspection, edge -of -waste
markers were observed to be in place around the entire perimeter of the landfill, as required.
7. The entire operations area surrounding the working face was found to be well managed. Adequate cover was in
place to prevent waste from becoming exposed, and to reduce windblown litter. The working face appeared to
be well compacted and small enough to be well controlled. No unacceptable waste was observed.
8. A few small piles of pushed up waste were noted beside the access roadway in the tipping area. Mr. Shelton
confirmed that all waste is covered at the end of every working day, and that these piles would be pushed back
into the working face and the entire operations area would receive daily cover.
9. Regarding windblown litter, Mr. Shelton stated that the county had hired a contractor to come in and clean the
entire facility of windblown litter. He explained that, once everything had been cleaned, facility staff have been
able to collect all windblown material and return it to the working face daily, as required. Mr. Shelton added
that this procedure was working better than attempting to have temporary workers handle the task. He further
stated that the county is considering having the contractor come on a regular basis to clear out windblown trash
— possibly as often as monthly.
Phase 4 Cell 4-B Inspection:
10. Operations are currently taking place in Phase 4, Cell 4-A. Phase 4 is designed in a dogleg shape that parallels
and wraps around the southeastern end of the Phases 1-2-3 disposal area. The dogleg is sloped with a high area
that bisects Phase 4 near the elbow curve so that the cell drains toward both the northern and southern ends in
opposite directions. Cell 4-A occupies the northern portion of Phase 4. Traffic barrels are staged in a line across
the high area to ensure that operations in Cell 4-A remain within the operational limits of the leachate collection
system of Sump 4-A. The side slope of Cell 4-A that abuts Cell 4-B appeared to be stable and well maintained,
and approximately 50' away from the barrels that mark the end of the northern cell.
11. Cell 4-B, which occupies the southern portion of Phase 4, appeared to be in good condition. The entire cell was

lined with gravel. No ponding water was observed in the cell, and no exposed waste or windblown litter was
noted.
12. Mr. Shelton opened a manhole at the southern end of the cell to reveal the valve system that had been put in
place to allow leachate from the active Cell 4-A to flow directly to the leachate pond and the stormwater
generated in the unused Cell 4-B to flow through the storm drain system. Mr. Maimon stated that the valve that
directs stormwater to the storm drain would be closed, thus directing all liquid in Cell 4-B to the leachate pond.
He further stated that the valve handle removed so that leachate could not unintentionally be directed outside
the leachate collection system once the valve was closed.
13. Underscoring the importance of separating the leachate from the stormwater in Phase 4, Mr. Maimon pointed
out the pump reading at sump 4-B that indicated more than 1.3 million gallons of stormwater had passed
through Cell 4-B since operations had begun in the northern section of Phase 4.
14. Mr. Shelton stated that he would not be ready to move operations into the new cell for a few weeks. Mr.
Maimon explained that once the decision is made to begin operations in Cell 4-B, he would provide written
certification to the Solid Waste Section that the leachate collection lines for the cell had been opened in
accordance with the approved plan. Please submit certification to Jordan Russ, Permit Engineer, via email
— jordan.russ(&ncdenr.gov. Operations and waste acceptance in Cell 4-B may not be initiated until
approval is received from Permit Engineer Jordan Russ.
