STATUS OF PERMIT:
Permit Amendment No. 5, Permit to Operate a Scrap Tire Disposal Facility was issued on December 10, 2012. The
Permit to Operate shall expire at the close of business June 30, 2016.
PURPOSE OF SITE VISIT:

OBSERVED VIOLATIONS
None
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and
any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. The tire monofill is permitted to receive rubber tires that have been processed (shredded, sliced, chipped or
otherwise cut) except for rubber tires, which do not require processing before landfilling.
2. The current franchise agreement with the City of Concord was signed on September 8, 2011.
3. The service area for the facility includes users from North Carolina, South Carolina, Virginia, West Virginia,
Georgia, Tennessee and Florida.
4. Observed operations of the tire processing area.
5. Observed operations of the tire monofill.
6. No unacceptable waste was observed.
7. A good vegetative cover was being maintained on seeded areas.
8. A stockpile of soil cover was being maintained near the working area.
9. Reviewed the permit, operations plan, groundwater and surface water sampling reports, tonnage records and scrap
tire certification forms.
