The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspected the following vehicles.
Tag#JB-7847, VIN# correct per the firm application.
Tag#YA143056, VIN# correct per the firm application.
Tag#YA130876, VIN# correct per the firm application.
Tag#HH-3874 (different tag# than listed on the firm application), VIN# correct per the firm application.
Tag#YA120657, VIN# 1HTSCAAL2WH563515, 950 gallons – this vehicle is not listed on the firm application
(was previously listed as Tag#AB-3223).
The tanks on the vehicle listed above appeared to be structurally sound & securely attached. Outlet covers were
on and no drips/leaks were noted. The lettering was correct on both sides of all of these vehicles except the
phone number listed on the vehicles (919) 639-2594 is not the phone number listed on the application.
2. The following vehicles were in the shop at this time & an inspection was not performed.

Tag#AB-3220 (the Tag, VIN & gallons associated with this vehicle will change in the future).
3. Vehicle Tag#AB-9020 was sold. Vehicle Tag#YA143056 is no longer in service.
4. Truck pumping logs were reviewed at this time. Ensure that all dates are recorded and also the name of
what WWTP was used for each disposal event.
5. Ensure that the firm application that is submitted to the Raleigh Central office at the end of this year has all the
correct information (Tag#, VIN#, gallons) for all vehicles to be used in the coming year.
6. Contact this office in the future to schedule an inspection for any additional vehicles to be added to this
permit.
