Kitty Hawk NC 27949 Telephone: (252) 489-3366 Email address: jonyounts@gmail.com PARTICIPANTS: Jeffrey Bullard DWM STATUS OF PERMIT: Active EXPIRES: 06/23/2018 PURPOSE OF SITE VISIT: Routine STATUS OF PAST NOTED VIOLATIONS: NA OBSERVED VIOLATIONS: Septage was applied outside of the permitted boundary on the east corner of field 2 in violation of the following rule. 15A NCAC 13B .0842(a)(12) all septage discharges, including aerial drift from discharges, shall be made within the permitted boundaries of the land application site; The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. The access roads leading to the disposal area were posted the required signage that was visible and legible. 2. The roads accessing the site were passable. 3. The disposal area boundaries were marked on the ground visibly and distinguishably from the surrounding landscape. 4. An area was observed on the east corner of field 2 where septage was discharged outside of the permitted boundaries. It appeared that the application of septage began too soon as the truck entered the permitted area. 5. Applications appear to be made in accordance with the nutrient management plan. 6. The crop described by the nutrient management plan was present and appeared to be maintained as prescribed. 7. pH not checked, no septage applications were made during the site visit. 8. No objectional odors where noticed. 9. Required setbacks appear to be met. 10. Land application records were made available for inspection. Required information appeared to be recorded.
