Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. The inspection was conducted in Mt. Airy, Surry County.
2. The following firm truck was inspected: Vehicle Identification Number (VIN) NC Tag Number Capacity (gallons) 1HTSDAAN3SH215750 YA146142 2000 3. The tanks appeared to be structurally sound and bolted to the frame.
4. Valves and access ports appeared to be secure. Caps were present on the access ports. The trucks were properly marked in accordance with the lettering requirements. The name of the company has been changed from “Donald Beck Jr. Septic Tank Service” to “Beck’s Septic”. “Beck’s Septic” was located on the tank. The former name of
the company was located on the door. 5. No leaks were observed.
