The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. The facility is open Monday — Friday 8:00 am — 4:30 pm according to the facility sign.
2. Records were on site and readily available.
3. A copy of current active permit was downloaded from the Section website during the inspection. I worked with
Brandon Smith on how to locate documents on our website. A copy of the operations plan was available. Site
staff will compare to the approved plan on CARA and ensure the most recent copy is available.
4. The facility is receiving approximately 300 tons of waste per week.
5. The training records were reviewed. There are two employees with current training. Recommend additional
employees receive training to ensure trained staff are on site while the facility is open.
6. Scrap tire records were reviewed. Ensure that customers have access to the most current forms and that the forms
are being filled out completely.
7. Waste screening records were reviewed. Waste screenings are being conducted by the scale house operator. I
recommend that the loader operator receive waste screening training to ensure unacceptable items that may not
visible on the top of a load are removed as they are observed.
8. The leachate is pumped and removed for proper treatment and disposal by a contractor.
9. Windblown waste was being managed well at the time of this inspection.
10. Scrap tires are collected and laced into transfer trailers.
11. Firefighting equipment was available at the WTS.
12. A load of waste was observed during the inspection. There was a large quantity of cardboard mixed in the load
along with some electronics.
13. The facility needs to be cleaned in accordance with the approved operations plan and permit to operate. The area
behind the push walls and loading areas should be cleaned as part of the regular cleaning of this facility. There
were some areas observed where the outer metal walls are corroding and need to be repaired. Measures to prevent
leachate or waste water from going out on the ground should be taken when cleaning the facility.
14. Facility staff was sprang the floor with a pressure washer while vehicles were dumping waste in the facility_
Washing down the facility while vehicles are moving in and out will track leachate outside of the facility. I
recommend you wait until no more vehicles are coming in to dump waste and all waste is removed from the floor.
15. There was some waste observed around the side areas at the entrance to the WTS. This waste should be collected
and placed in the trailer before close of business each day.
16. The trailer loading area should be cleaned daily as well. The leachate drains were covered with dirt and waste at
the time of this inspection. The drains should be checked periodically to ensure that the lines are clear and free
flowing.
17. The leachate collection tank located outside the WTS was inspected and appeared to be working at the time of this
inspection. I recommend testing the alarm periodically to ensure that the overflow prevention system is working
property as well.
18. The yard dog used to move the trailers in and out of the loading area continues to leak. Measures to contain the
leak need to be taken by either fixing the vehicle or placing a drip pan beneath the vehicle. This area needs to be
cleaned.
19. Oil was observed on the concrete pad beneath and around the used oil collection tank as well as the top and sides.
Spills should be cleaned as they occur or as soon as they are observed. Recommend that you check the drain pan
on top of the tank to ensure it is properly cleaned and installed on the tank to prevent spillage.
20. The eroded area observed near the used oil collection area that leads to an outfall needs to be repaired.
21. Freon is removed from the scrap white goods by the contractor.
22. Refrigerators and other Freon containing items stored in the white goods area were on their sides. Freon
containing recyclables should be stored in an upright manner and separated and labeled as the Freon is removed.
23. Power equipment stored in the whitegoods and metal storage area should be checked to ensure fuel and oil has
been removed to prevent spills.
24. The electronics are received, placed in card board boxes, palletized and stored in a transfer trailer before removal
by the contractor.
25. The county has established a pesticide and herbicide container collection site at the facility.
26. I recommend that all staff, permanent or temporary, that will be working at the facility read the permit
and approved operations plans.
27. Corrective measures need to be underway.
