PURPOSE OF SITE VISIT: Comprehensive STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 ADDITIONAL COMMENTS 1. The facility hours of operations are Monday — Friday 8:00 — 4:00. 2. The permit and operation records were readily available for inspection. 3. Training records were reviewed. The facility currently has four trained staff. 4. The 5 year permit to operate was issued March 14, 2012. The permit expires on August 1, 2015. Pursuant to 15A NCAC 13B .0201(g), no later than February 2, 2015, contact the Section for permit review. 5. The waste type observed during the inspection was mostly correct for the facility. There were some electronics and containers of waste cooking oil observed during a waste screening conducted by the transfer station operator on two loads from the county convenience sites. The oil and electronics were being removed during the inspection for proper handling. 6. Windblown waste was being managed well. 7. Fire extinguishers were present at the transfer station. 8. Daily cleaning of the transfer station should include the areas behind the push walls and the trailer loading area per section 2.6.5 of the operations plan. Daily cleaning should eliminate waste residue buildup behind push walls and in the trailer loading area. 9. The weekly cleaning of the transfer station should occur in accordance with section 2.6.6 of the operations plan to include the areas behind the push walls and the trailer loading area. 10. I recommend that you check the truck used for moving trailers in and out of the loading bay for petroleum leaks. 11. The waste around the electronics collection container should be cleaned up daily. 12. Scrap tires are being placed into a trailer. Documents were available for review. 13. Electronics are collected, packaged in boxes and placed in a storage trailer until removal by the contractor. There were some electronics outside the trailer on the ground that needs to be managed better. 14. Lead acid batteries are being collected on a pallet adjacent to the electronics. Batteries were observed on the ground on their sides with electronics on top of them. Ensure batteries are palletized upright to prevent leakage. 15. Leachate collection drains should be clean and clear in a free flowing condition. One of the drains in the trailer loading area was covered with dirt and residue. Ensure that the drain cover is cleaned and maintained. 16. The leachate containment tank is pumped and hauled by a contractor. The leachate storage tank outside the WTS had approximately 6 inches of freeboard. Ensure that all pumps and alarms for the system are operating properly. 17. The trailers used for hauling waste the landfill were observed to have holes in the back gate and sides. Waste was visible through the holes on the trailer being loaded, as well as a full one outside the WTS waiting to be hauled to the Sampson County Landfill. There were no leaks observed from the loaded trailers, however, loaded trailers have the potential to leak while awaiting transport. Continue to monitor full trailers parked outside the leachate containment system and promptly contain and stop leaks and cleanup and dispose of any leachate contaminated soils. Recommend that you contact the waste hauler and bring the holes in the trailer to their attention. 18. The area where used oil is being collected needs to be cleaned up where spill have occurred. Barrels containing waste oil and used oil filters should be placed on pallets or concrete and off the ground with lids secure to assist in the prevention additional releases. Spills should be cleaned up as soon they occur or are noticed. Clean up of this area needs to start right away. 19. White goods are being managed on the concrete pad. There was a couple of old televisions observed mixed in with the metal that need to be removed. 20. Grass and weeds grown up around pad should be mowed so that trash can be cleaned up and disposed of. 21. Digital photos were taken during this audit.
