PURPOSE OF SITE VISIT: Partial Audit STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. The facility hours of operations are Monday — Friday 8:00 — 4:00. 2. Please ensure that a copy of all operating records are maintained onsite and readily available for inspection. 3. The permit expired on February 11, 2010 and the renewal application submitted on November 10, 2010 and again on March 1, 2012. A permit has not been issued for this site. The operation of a facility without a valid permit can result in potential enforcement actions. I recommend that you enact a method of monitoringyourpermits to ensure that the permit renewal is submitted in sufficient time for review to prevent the expiration of the permit in the future. I also recommend that in the future you contact the permitting branch and or the regional specialist prior to expiration of a permit for a status update. 4. There was no waste on the tipping floor at time of the audit. 5. Windblown waste was being managed well. 6. Fire extinguishers were present at the transfer station. 7. Daily cleaning of the transfer station should include the areas behind the push walls and the trailer loading area per section 2.6.5 of the operations plan. Daily cleaning should eliminate waste residue buildup behind push walls and in the trailer loading area. 8. Ensure that drains are clean and clear in a free flowing condition. 9. Weekly cleaning of the transfer station should occur in accordance with section 2.6.6 of the operations plan. 10. I recommend that you check the truck used for moving trailers in and out of the loading bay for petroleum leaks. It. The waste around the electronics collection container should be cleaned up daily. 12. Scrap tires are being placed into a trailer. 13. Electronics are collected, packaged in boxes and placed in a storage trailer until removal by the contractor. 14. The leachate containment tank is pumped and hauled by a contractor. 15. There were no leaks observed from the loaded trailers, however, loaded trailers have the potential to leak while awaiting transport. Please monitor full trailers parked outside the leachate containment system and promptly contain and stop leaks and cleanup and dispose of any leachate contaminated soils. 16.
