(On April 13, 2011 the Avery County Transfer Station (06-02T) was incorporated, by amendment, into the permit for the Avery County Construction & Demolition (C&D) Landfill (06-03).) PURPOSE OF SITE VISIT: Partial Inspection Limited to the Construction and Demolition (C&D) Landfill. STATUS OF PAST NOTED VIOLATIONS: N/A. OBSERVED VIOLATIONS: There were no violations observed. The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. The C&D facility is permitted to receive solid waste generated in Avery, Burke, Caldwell, McDowell, Mitchell, and Watauga counties in North Carolina, and, Carter, Johnson, and Unicoi counties in Tennessee. 2. The C&D landfill currently accepts approximately 25-tons of waste per day. 3. Wastes were actively being accepted during the inspection. No unacceptable wastes were observed at the landfill. Edge -of —waste marker on the C&D landfill. 4. Landfill gas monitoring well GP-1 on the (L), groundwater monitoring well MW-1 on the (R). A random inspection of groundwater and landfill gas monitoring wells (Photo #4) found them to be properly secured, labeled and maintained. 7. Passive landfill gas vents have been installed on the closed out cells of the landfill. (Photo # 5) It is recommended that these passive landfill gas vents be labeled with IDs to facilitate any maintenance, repairs and or monitoring that maybe needed. 8. The stormwater basins have ample freeboard and do not appear to need to be dredged of sediments at this time. (Photos #6) 9. The vegetative cover on the closed and active cells is well established. No evidence of any excessive erosion was observed. 5. Passive landfill gas vent (typical) on the C&D landfill. 6. Looking (W) along the length of Sediment Basin 10. Waste screening records were reviewed. These records are maintained in the office in the transfer station. Wastes are screened at three to four times a week. 13. Handheld fire extinguishers are on site and the Green Valley VFD is available for fire control. 15. The facility is served by a water supply well that is located behind (to the southwest) of the transfer station. 16. The following operator training was completed: Eric Foster — MOLO, certification expires 6/27/2016 Shane Eggers — Landfill Operations Specialist, certification expires 4/27/2016 Please contact me if you have any questions or concerns regarding this inspection report.
