The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspected the pump truck, tag#YA109220, VIN#1FDXF80C5WVA21424 – this is slightly different than the
VIN# listed on the firm application. The tank appears structurally sound & securely attached. Outlet covers
were on and a slight drip was noted but possibly was from condensation (ensure that the valve is not leaking).
The lettering on the vehicle is at the correct height on both sides of the vehicle. However, some of the info
displayed on the vehicle did not coincide with the info on the firm application – the name on the vehicle is
“Travel Resorts of North Carolina, LLC” and the phone number displayed is “910-420-3838”.
Corrections were made to the firm application & this info was submitted to the Raleigh Central Office.
Ensure that accurate info is listed on the firm renewal application this winter – particularly the correct VIN#,
firm name & phone number as displayed on the vehicle.
2. Discussed the required info that shall be maintained (specific to the activities of this firm) for the truck pumping
logs - as the info is not currently being recorded properly. An example truck log was given to Mr. Jones.
3. A company representative will attend a continuing education class later this year to get the required training
hours.
