STATUS OF PERMIT:
A Permit to Construct/Operate a Permanent Household Hazardous Waste Facility was issued to Ashe County on
November 5, 2009. The permit will expire on November 5, 2014.

The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and
any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
ADDITIONAL COMMENTS .
1. The facility receives HHW from Ashe County only. The facility also has a convenience center for the collection
of electronics, used oil, oil filters, vehicle batteries, cardboard, aluminum cans, glass bottles, newspapers, plastic
bottles, scrap tires, white goods, and scrap metal.
2. The permit and operations plan were reviewed. Both documents were current.
3. The facility performs annual training for employees. These documents are kept at the facility and made available
when requested.
4. Facility emergency equipment is inspected daily. Daily records were verified. Equipment includes gloves, hard
had, aprons, mask and cartridges, and the eye wash station.
5. All HHW is stored in a steel, trailer -type box with three separate compartments. The first compartment is
designated for storage of oil based paint and flammable liquids, the second compartment stores latex based paint,
and the third compartment stores insecticides, herbicides and antifreeze. All compartments were well
maintained.
6. A new building had been placed at the facility since the last inspection for the collection of fluorescent bulbs.
7. All facility storage compartments are equipped with a secondary containment area beneath the floor to prevent
spills from being lost. All sump outlets leading from the secondary containment area were capped at the time of
inspection.
8. The facility was equipped with a dry chemical fire suppression system. Mr. Turner stated that the fire
suppression system had been inspected. However, the inspection tag was not present at the time of inspection.
Please ensure that the inspection tag is present in the future.
9. All drums were marked with the date each period of accumulation began as required by the approved operations
plan.
10. As requested on the last inspection report, each HHW compartment should be equipped with a functioning
locking mechanism. The approved operations plan states that the IIHW storage building will be secured
by locked doors. No locks were in place on the doors at the time of inspection.
11. The facility is surrounded by chain -link fence and a gate is provided at the entrance to prevent unauthorized
access.
12. Proper signage.was provided at the entrance to the facility.
13. Access around the facility was free of obstacles.
14. Facility manifests were reviewed. EcoFlow, Inc. collects paint and other assorted HHW. EcoFlow, Inc. last
performed a pick-up on November 12, 2012.
15. Pesticides and insecticides are collected by the North Carolina Department of Agriculture and Consumer
Services. The NCDA&CS picked up 402.9-pounds of waste on August 20, 2012.
