Expires August 09, 2021. Pursuant to 15A NCAC 13B .0201(g), no later than April 27, 2021, the owner or operator must submit a request to the Section for permit review and must submit updated facility plans meeting the requirements of Rule 0.565 and .0566. PURPOSE OF SITE VISIT: Comprehensive Facility Compliance Inspection. STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS: None The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 4 ADDITIONAL COMMENTS: 01 – Facility has required signage. 02 – Locking gates and fencing are used for security. 03 – Roads are constructed of all-weather materials and maintained well. 04 – Edge of waste markers present. 05 – Cover logs were reviewed during this inspection. LCID has been covered recently. 06 – Adequate amount of earthen cover material on-site and ready for use. 07 – Facility now has more than one working face. If facility is using more than one working face, ALL must meet cover requirements that ALL waste must be covered within 30 days. 08 – The East side slopes are in need of maintenance. The slope is 1:1.5 which has caused sever erosion during some earlier heavy rainfall events. These areas need to be repaired/graded. 09 – I would suggest that you start working towards the required 3:1 side slope to prevent the sever erosion. 10 – There was minimal non-conforming waste noted during this inspection. Please remove and remind drivers of the type waste this facility is permitted to receive. 11 – Be sure not to let sapling trees to grow in the landfill permitted area. This would include the side slopes. 12 – Incoming load receipts are available. 13 – COVID-19 recommendations were followed during this inspection. 14 – Digital pictures were taken during this inspection with several attached to this report.
