Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 3 ADDITIONAL COMMENTS 1) Pumper vehicle(s) inspected: TRUCK LICENSE PLATE # VIN # TANK CAPACITY (gal) YA 160340 1FUBC5CVX7HY68208 4,000 2) All required information was present on both sides of the vehicle. The letters for the phone number and the firm name except for the “LLC” were three (3) inches or greater in height. The letters for the permit number, town name, and “LLC” were less than the required three (3) inches in height. Received a text message from Michael on 11/09/2020 that had pictures showing that the letters for the permit number, town name, and “LLC” had been redone to be at least three (3) inches in height. See pictures below. 3) Tank was made of steel and appeared to be structurally sound. 4) The tank was affixed to the frame of the truck. 5) Access ports appeared to be in working order and properly sealed. 6) Valves appeared to be in working order. Caps present on the hose ports. 7) No leaks noticed. 8) Based on the information above, the truck passed inspection. Driver side of truck. Picture taken on 10/23/2020 by Chester.
