Comprehensive Audit
NOTICE OF VIOLATION(S):
1. Failure to maintain groundwater monitoring wells in accordance with the well construction standards per 1 SA
NCAC 2C .0108(o) All wells shall have permanently affixed an identification plate which shall contain the
following information:
(a) Well contractor name and certification number;
(b) Date well completed;
(c) Total depth of well;
(d) A warning that the well is not for water supply and that the groundwater may contain hazardous materials;
(e) Depth(s) to the top(s) and bottom(s) of the screen(s); and
69 The well identification number or name assigned by the well owner.

You are hereby advised that, pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each
violation of the Solid Waste Statute or Regulations. For the violation(s) noted here, you may be subject to enforcement actions including
penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be
necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

1. The facility is secured by a gate.
2. Figure 1 shows the approximate locations of the groundwater monitoring wells, landfill gas monitoring wells, and the
surface water monitoring locations.

3. All of the groundwater monitoring wells were secured with padlocks. (The locking cap hinge on groundwater
monitoring well MW-9D is broken and needs to be repaired. Photo #5)
4. "Edge of waste" markers made of 2-inch white PVC pipe have been installed. (Photo #1)
5. The top of the landfill has a good vegetative cover that is well maintained. The landfill had recently been cut. (Photos
#1 and #2)
6. There was no evidence of any excessive erosion, ponding of water, or any settlement of the cap.
7. The landfill was free of trees and woody vegetation.
8. The locking cover to MW-9D was broken and must be repaired. (Photo #5)

well is not fitted with a stopcock valve or a quick connect
coupling on the cap, which allows for accurate landfill gas
measurements.)
6. Landfill gas monitoring well LFG #5. (Note what
appears to be a bentonite seal at the base of the PVC riser.)
8. Looking down the riser of LFG well #2A. (Note that the
well is not fitted with a stopcock valve or a quick connect
coupling on the cap, which allows for accurate landfill gas
measurements.)
9. None of the landfill gas monitoring wells were fitted with a stopcock valve or a quick connect coupling on the
cap, as required by the Division of Waste Management, Solid Waste Section, Landfill Gas Monitoring Guidance -
November 2010. (Photos #7 and #8)
10. Required semi-annual analysis of water samples from nine of the ten existing groundwater monitoring wells and surface
water monitoring from two locations was last completed in May 22, 2013, by Scarlett Geophysical Consulting, P.C.
11. All of the groundwater and surface water samples were analyzed for the Federal Appendix I list of volatile organic
constituents and for the eight RCRA metals.

12. The Rutherford County Sheriff's Department has a shooting range on the property. (Figure 1)
13. The access road was maintained in good condition.
