PURPOSE OF SITE VISIT: Comprehensive Inspection STATUS OF PAST NOTED VIOLATIONS: NA OBSERVED VIOLATIONS: None Page 1 of 3 FACILITY COMPLIANCE INSPECTION REPORT Waste Management Division of Waste Management : .t ENVIRONMENTAL QUALITYr Solid Waste Section The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. ADDITIONAL COMMENTS 1. Mr. Gentry stated that Jimmy Lewis was the Operator of the facility. 2. Per 15A NCAC 13B .0566(4), "Adequate soil cover shall be applied monthly, or when the active area reaches one acre in size, whichever occurs first." Landfill material needs to be adequately covered with soil at least monthly, including the working face. 3. Solid waste shall be restricted to the smallest area feasible and compacted as densely as practical into cells. There is waste exposed all around the perimeter of the landfill. 4. Remove any unacceptable materials, i.e. metal pipe, house hold trash, tires, etc. Unacceptable waste should be removed when loads are dumped at the LCID. 5. There are scattered piles of debris outside of the LCID waste area. These must be moved to the LCID and covered. 6. Side slopes must be graded and covered with soil such that a 3:1 slope ratio is obtained. 7. Ensure the landfill cap is graded such that it does not hold water. 8. Areas not in use should be sown with grass seed or ground cover sufficient to restrain erosion. 9. Permanent, easily seen, in line of sight, Edge of Waste markers are to be established to mark the less than two acre area for disposal. The edge of waste must be a minimum of 100 feet off of any property line. 10. It is recommended that a fire lane be established around the entire waste unit to allow emergency vehicle access. 11. Access roads are of all-weather construction. 12. The facility is secured by a metal entrance gate. 13. The facility has proper signage. 14. Corrective actions are needed as a result of this Audit.
