STATUS OF PAST NOTED VIOLATIONS: N/A
OBSERVED VIOLATIONS: None.
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and
any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

Mr. Geiss has been granted an extension on his certification until the SWANA training session on 6/04/13.
3. Copies of the current Permit to Operated and the Operations Plan are maintained on site.
4. Proper signage is displayed at the entrance the landfill.
5. The facility is permitted to accept approximately 640 tons of waste daily for a maximum total of
approximately 200,000 tons per year.
6. The facility is currently accepting approximately 70 tons of waste per day.
7. The landfill was actively receiving waste during the audit. The operational area/working face was small and
well maintained.
8. There are four monitoring wells in the groundwater monitoring networks (MW-1, MW-2, MW-3, and MW-4)
that are sampled twice a year. Ground water samples were last collected for analysis on October 24TH and
25TH, 2012.
9. There are two surface water monitoring stations (SW-1 and SW-2). Surface water samples were last collected
for analysis on April 13, 2013.
10. There are six landfill gas monitoring wells (MMW-1 through MMW-6). Landfill gas monitoring is conducted
semi-annually by Landmark Environmental Systems, LLC. The most recent monitoring event occurred on
May 20, 2013 when wells MMW-1 through MMW-5 were sampled. Two site buildings (scale house and the
maintenance building) are also monitored semi-annually. (See table below.)

11. Waste screening records were reviewed. Incoming waste loads are randomly screened two to three times a
week.
12. Several sections of movable fencing are used to capture and control windblown litter. There was very little
windblown material observed during this inspection.
13. There edge -of -waste markers were in place at the time of the audit.
14. The landfill has been subjected to recent localized heavy rains. Stormwater is well managed by the use of
diversion berms and maintaining proper slopes.
15. As a result of the recent rain events, six separate leachate seeps have developed on the northeast slopes of the
landfill. Three of these seeps have already been repaired by excavating them and "turning them back into the
landfill". The remaining three seeps should be repaired within a week. No leachate move outside of the
limits of the area of waste.
16. A "roll off box" is staged near the working face for the collection of scrap tires and other unacceptable wastes
that the landfill staff removes from the waste stream.
17. There was very little windblown litter observed.
18. The access roads to the C&D landfill were well marked and well maintained.
19. The access road to the groundwater and landfill gas monitoring wells can become muddy near the sediment
basin. Please ensure that the access road remains passable at all times.
20. No woody vegetation or trees were observed on the landfill.
21. Grass cover on the south and east slopes is well established.
22. All ground water and landfill gas monitoring wells were properly identified with "well tags" and secured with
padlocks.
