This is a Notified site and is recorded in Book 708, Pages 99(7), with the letter of approval being sent from the Solid
Waste Section on January 12, 2009.
PURPOSE OF SITE VISIT:
Comprehensive Inspection

The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS:
1. There is a load of what appears to be ditch cleaning material at the site. There are a lot of unacceptable wastes
including plastic bottles, cans, etc. mixed in with the soil. These items should be removed when the load is
dumped at the site.
2. There are some areas on the covered waste that need ground cover established to prevent erosion.
3. There does not appear to have been any land clearing debris brought to the site since the last inspection.
4. The site is bermed with soil to show the less than two acre location for the LCID.
5. There is a cable across the entrance to prevent unauthorized access to the site.
6. There is a sign with the name of the site and an emergency contact number at the site.
7. It appears that all setbacks are being met.
8. The site looks very good.
