PURPOSE OF SITE VISIT: Partial Audit STATUS OF PAST NOTED VIOLATIONS: NA OBSERVED VIOLATIONS NONE The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 ADDITIONAL COMMENTS: 1. Larry Chalker is the new Public Works Director. 2. There appeared to be many old piles of leaves with vegetation growing on them. These are to be included in the less than two acre staked off area. A yard waste notification only allows for less than 6000 cubic yards, to be processed and stored on a quarterly basis. Between the old piles of leaves around the site and the depth of the leaves in many areas of the site, the 6000 cubic yards allowable, may be being approached. If you are approaching this limit, with all of the old piles, you must notify the Section to get a permit. 3. The topography of this site is not conducive to the ease of the required management of the leaves. It is very strongly recommended that another site be found and this site no longer used. This site must be cleaned up. 4. There appeared to be a lot of `trash' included in the leaf piles. The incidental `trash' at the site needs to be removed on a more frequent basis. 5.
