Solid Waste Director STATUS OF PERMIT: Active — Permit to Operate issued August 22, 2006 Permit expires: August 22, 2011 Permit renewal request submitted to Section: April 22, 2011 Updated Operations Plan dated September 5, 2011 PURPOSE OF SITE VISIT: Site visit to follow-up with previous DWQ findings; partial SWS inspection STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS: None The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. On site on October 11, 2011 to review stormwater outfall locations, drains, and erosion control measures as related to the facility DWQ discharge permit. Susan Wilson, NCDENR Division of Water Quality — Surface Water Protection, present during inspection. 2. The precautionary measures taken by the County regarding the facility stormwater drains (drain sleeves) were in place but required management (loose fit or clogged by particulate). It was stated (SKing) that the maintenance of these sleeves was to be added as a job requirement for specific employees to ensure regular cleanout and checks of these drain systems. 3. No issues were noted at the White Goods or tire collection areas of the facility. No windblown issues were noted. 4. The tipping floor was manned and waste screening was being conducted. No issues noted. 5. Household Hazardous Waste (HHW) such as insect repellants, herbicides, cleaners, dyes, and antifreeze, can be segregated for treatment, recycling, or disposal, separate from the incoming waste stream. However, in order to accept, collect, and store these types of HHW the facility should have an updated Operations Plan and modified permit which details how this material is managed, stored, and treated. Please note that to be defined as "household" waste and thus exempt from federal hazardous waste regulation, the waste must be generated by individuals on the premises of a residence for individuals (a household) and composed primarily of materials found in wastes generated by consumers in their homes. 6.
