The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS 1. The Septage Detention and Treatment facility (SDTF) consisted of one 100-gallon, two 4,000-gallon, three 1,000-gallon, one 6,000-gallon, one 10,000-gallon and one 12,000-gallon tanks. The nine tanks have a total capacity of
39,100 gallons for the storage of untreated and treated waste. In addition, one 6,000-gallon dewatering drum and one 30-yard dewatering box is on site.
2. No leaks or spills from the tanks were observed; however, there was leachate at the lower section of the facility. There is a sump pump located where the dewatered sludge and wood chips are mixed was in operation during the inspection. Continued upkeep and maintenance is necessary to ensure that leachate does not flow off of the property. Additional actions that are also required and has been previously discussed are: A. A daily safety checklist is to be maintained by Triple T that includes inspection of the tanks for leaks and the sump pump. The checklist must also include any corrections made to prevent spills or leaks and any additional
notes that are necessary for clarification. This is to be implemented immediately. B. A berm needs to be constructed below the facility to prevent overflow into the culvert.
3. Appropriate signage was in place at the entrance to the access road. There is also a gate to prevent un-authorized access.
4. No odors were detected at the property lines or on property off site. 5. All weather roads are maintained for access to the site.
6. The area around the tanks were well maintained. 7. Solids from the dewatering system are still currently being mixed with wood chips and taken to the Foothills Regional MSW Landfill instead of composting. Leachate from the pile is collected in a tank. 8. Kim Pierson is no longer with the company which may require a permit revision since her name is listed on the
