John College – NCDEQ-DWM-SWS-FOB STATUS OF PERMIT: Active, Life of Site Permit, Issued February 20, 2017. Combined with 2803-CDLF-1995. PURPOSE OF SITE VISIT: Comprehensive Facility Compliance Inspection. STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS: None The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 5 ADDITIONAL COMMENTS: 01 - Life of Site Permit was issued 2/20/2017 as a combined permit with facility #2803- CDLF and was reviewed along with the operations plan and contingency plan during this inspection. Facility is permitted to receive waste from Dare Co. Passed by the Dare Co. Commissioners on August 02, 2004. MSW is transferred to 0803-MSWLF in Bertie Co. 02 - Facility has required signage. Locking gates and fencing for security. 03 - Interior roads are constructed of all-weather materials and well maintained. 04 - Incoming load tickets were reviewed during this inspection and had required information. 05 - Waste screening records were reviewed during this inspection and found to be up to date. 06 - Leachate removal records were reviewed and the last record was dated 4/5/2021. 07 - Leachate tank has plenty of storage until next removal occurs. 08 - The leachate drains could use attention. Not currently overflowing, but has the potential to stop up with the amount of trash in them. 09 - There was quite a bit of windblown trash noted today. Some was new and some was old. No one picking up windblown trash during this inspection. Please remember that ALL windblown trash is required to be picked up by the END OF THE WORKING DAY. 10 - The load out bays, were full of spilled over MSW on both sides of the trailers in both bays. This was during working hours. It was suggested to Melvin Cuthbertson that if the spill over MSW was cleaned up between trailers, this would help reduce the windblown trash issue and be more sanitary. 11 - Facility has 6 Certified Transfer Station Operations Specialist (TSO): Mike Chesson, expires 5/18/2021, Kelvin Wrighton, expires 11/5/2021, Jonathan DeWeese, expires 11/15/2021, Deborah A. Equils, expires 11/3/2022, Thomas Tillett, expires 10/9/2023 and Doug Huff, expires 5/4/2024. Please remember that a that a ACTIVE TSO MUST BE ONSITE DURING ALL HOURS OF OPERATION. 12 - Facility had 6 loaded trailers and 2 trailers in the load out bays that were almost full. 1 empty trailer was in line. No leaks were noted on the loaded trailers during this inspection. 13 - The tipping floor had a lot of MSW being loaded. Behind push walls was free of MSW. 14 - All loaded trailers had netting style tarps on them to help with windblown trash. 15 - Covid-19 recommendations were followed during this inspection. 16 - Digital pictures were taken during this inspection and some are attached to this report.
