PURPOSE OF AUDIT:
Compliance Audit (to ensure that the Land Clearing and Inert Debris (LCID) Landfill is in compliance with 15A North
Carolina Administrative Code (NCAC) 13B .0563, .0564, and .0566)
NOTICE OF VIOLATION(S):
None
You are hereby advised that, pursuant to N.C.G. S. 130A-22, an administrative penalty of up to $15,000 per day maybe assessed for each violation of
the Solid Waste Statute or Regulations. For the violations) noted here, you may be subject to enforcement actions including penalties, injunction
from operation of a solid waste management facility or it solid waste collection service and any such further relief as may be necessary to achieve
compliance with the North Carolina Solid Waste Management Act and Rules.

STATUS OF PAST NOTED VIOLATIONS:
None
AREAS OF CONCERN AND COMMENTS:
On October 14, 2009, John Patrone met with Kerry Venable and Kerry Venable II and conducted a compliance audit of the
Land Clearing and Inert Debris Landfill (SWS ID No. N0677) located on Preston Road in Belews Creek, Forsyth County.
I. The working face (where LCID material is deposited) must be limited to the smallest area feasible.
2. The LCID material must be adequately covered with soil on a monthly basis. The slope should be graded and
covered such that a 3:1 slope ratio is obtained or a ratio of which no indication of erosion or runoff is observed.
3. The facility appeared to have adequate borrow material available for soil cover.
4. Ensure that the LCID landfill does not encroach upon setbacks associated with the adjacent property as per 15A
NCAC 13B .0564(9)(b), "The facility shall meet the following buffer requirements: 100 feet from the disposal area
to property lines, residential dwellings, commercial or public buildings, and wells."
5. The facility is secured by a locked gate.
6. The access road is of all-weather construction.
7. The facility has proper signage.
8. A copy of the Land Clearing and Inert Debris Landfill Notification will be mailed to Kerry Venable.
9. A copy of the rules pertaining to Notified LCID landfills will be mailed to Kerry Venable.
