PURPOSE OF AUDIT:
Comprehensive Audit (to ensure that the Land Clearing and Inert Debris (LCID) Landfill is in compliance with 15A North
Carolina Administrative Code (NCAC) 13B .0563, .0564, and .0566).
NOTICE OF VIOLATION(S):
None
You are hereby advised that, pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day maybe assessed for each violation of
the Solid Waste Statute or Regulations. For the violation(s) noted here, you may be subject to enforcement actions including penalties, injunction
from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve
compliance with the North Carolina Solid Waste Management Act and Rules.

STATUS OF PAST NOTED VIOLATIONISJ:

AREAS OF CONCERN AND COMMENTS:
On October 29, 2009, John Patrone met with Gina Cecile and conducted a comprehensive audit of the Land Clearing and
Inert Debris Landfill located on Ridge Road in Pfaf flown, Forsyth County.
I. The facility entrance (via gravel road) is located on the right, just after the New Bethel Baptist Church. A fence
post labeled with No. 6350 is located near the intersection of Ridge Road and the facility entrance.
2. The LCID landfill footprint is currently —'/2 acre. The Cecile's property is 38.32 acres.
3. The working face and slope (where LCID material is deposited) must be limited to the smallest area feasible.
4. The LCID material must be adequately covered with soil on a monthly basis. The slope should be graded and
covered such that a 3:1 slope ratio is obtained or a ratio of which no indication of erosion or runoff is observed.
5. The facility appeared to have adequate borrow material available for soil cover.
6. The facility is secured by means of a locked gate.
7. The access road is of all-weather construction.
8. The facility has proper signage.
9. A copy of the Land Clearing and Inert Debris Landfill Notification was given to Gina Cecile.
10. A copy of the rules pertaining to Notified LCID landfills was given to Gina Cecile.
