Land Clearing and Inert Debris Landfill Notification approval letter dated April 1, 2002 was issued from the SWS to Gerald Peddychord. Current status of Land Clearing and Inert Debris Landfill Notification is OPEN. PURPOSE OF AUDIT: Update Comprehensive Audit (to ensure that the Land Clearing and Inert Debris (LCID) Landfill is in compliance with 15A North Carolina Administrative Code (NCAC) 13B .0563, .0564, and .0566). NOTICE OF VIOLATION(S): None. You are hereby advised that, pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Statute or Regulations. For the violation(s) noted here, you may be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE AUDIT REPORT Division of Waste Management Solid Waste Section Page 2 of 2 STATUS OF PAST NOTED VIOLATIONS: None. AREAS OF CONCERN AND COMMENTS: On April 21, 2009, John Patron met with Gerald Peddychord and conducted an update compliance audit of the Land Clearing and Inert Debris Landfill located at 5910 Belews Creek Road in Walkertown, Forsyth County. 1. LCID material has been removed from atop the graded portion of the landfill and placed along the slope. 2. Mr. Peddychord was reminded that LCID material should not be stockpiled atop the graded portion of the landfill. 3. Mr. Peddychord was told that LCID material must be adequately covered with soil (including all slopes) on a monthly basis. 4. The slope along the side of the LCID landfill should be graded such that it does not impound water. 5. The mulch, on -site, should be routinely turned and placed in a windrow. An effort should be made to remove/use the mulch as soon as possible. 6. The site is secured by a gate and chain. Mr. Peddychord was asked to place a lock on the gate. 7. Mr. Patrone told Mr. Peddychord that the SWS would re -inspect the LCID landfill and that he must ensure that LCID material is routinely covered and that the slopes along the side (of the landfill) are properly graded.
