Failure to operate as permitted may result in the Department suspending or revoking this permit, initiating action to enjoin the unpermitted operation, imposing administrative penalties, or invoking any other remedy as provided in Chapter 130A, Article 1, part 2 of the North Carolina General Statutes. This permit and registration expires on

Waste Management. The 30.17-aCre Site sha= be divided into four fie!ds, FieId 5A
(8.01 acres), Field 5B (4.88 acres), Fieid 6A (9.74 acres) and FieId 6B (7.54 ac「es)・ The
30-daywaiting pe「iod between the last application ofseptage and the ha「vest ofthe
crop shaiI be met by altemating septage app"cations′ the waiting period and the
ha「vest ofthe c「op between the four pe「mitted disposal fields. All discharges sha=
occur within the permitted a「ea and sh訓be consistent with the crop rotation in the
approved pIan.

4. The issuance ofthis permitdoes not precludethe Permitteefrom complyingwith any

etc. is prohibited,
13. This permit sha一一become void unIess the land application activities are car「ied out in
accordance with the conditions ofthis permit and in the ma=ner aPP「OVed bythis
Division. Noone otherthan the Pe「mittee oran empIoyee ofthefi「m named in this
pe「mit sha= discharge septage at this site without prior appropriate =Otification and
written approvaI from the Division.
14. Priorto anytransferofthis !and′ a nOticesha11 begiventothe newownerthatgivesfuli
deta=s ofthe mate「iaIs applied or incorporated atthis site. The Division shall be
notified prio「to site cIosure. This permit is non-tranSferable.

7. This faciIity consists of two lO′000 gallon above ground metai tanks with a totaI
CaPaCity of 20′000 gallons for the storage of domestic septage and grease septage,
Storage ortreatment ofany iiquid or solid wastes otherthan those specified in the
approved appiication forthis permit is prohibited.
8. No one otherthan the Permittee or an empIoyee ofthe firm named in this permit
may discharge septage at this detention and treatment fac冊y uniess the Division
is notified and approvai is granted by the Division. Only Septage Management
Firms permitted to operate in North Carolina may receive autho「izations to
discharge at this detention and treatment fac冊y.
