STATUS OF PERMIT:
A Permit to Operate a Large, Type 1 Solid Waste Compost Facility was issued to the City of Winston-Salem on
October 6, 201.0. This permit shall expire on October 6, 2010.

OBSERVED VIOLATIONS

The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties maybe subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and
any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
ADDITIONAL COMMENTS
1. This facility is a Large, Type I Solid Waste Facility. Compost is produced from brush leaves, limbs, and grass
clippings. Mulch and boiler fuel is produced from land clearing debris, untreated and unpainted wood, and
pallets.
2. Temperature records were reviewed for January 2011 to September 2011. Records showed that temperatures
were recorded above 131°F. Multiple temperatures were observed at or near 169F. The compost windrow
should be turned when such temperatures are observed to prevent excessive heat buildup.
3. Tonnage records were reviewed from January 2011 through August 2011. Records showed that 16,425.51 tons
of material was received during this time period.
4. Signs placed at the entrance to the facility were correct.
5. A gate is located at the entrance to the facility to prevent unauthorized access.
6. A scale house attendant was on duty during the inspection.
7. A large windrow of double -ground mulch was located on site at the time of inspection. This material was placed
within a windrow south of the primary receiving area for trees and brush.
S. One compost windrow was located on site at the time of inspection.
e It appeared that the dimensions of the windrow were within the size limits outlined on the Permit to
Operate. Permit Condition #8 requires that the windrow dimensions for active composting be limited
to a maximum of 15-feet high x 30-feet tall.
u Freshly ground material is added to the rear of the compost windrow (west end), while final material
is taken from the front of the windrow (east side).
9. A significant amount of single -ground material was on site at the time of inspection which was being loaded into
a trailer to be shipped out as boiler fuel.
10. No unacceptable waste streams were observed within the compost area or receiving areas during the inspection.
11. No significant odors were observed at the time of inspection.
12. No erosion or standing water was observed on site at the time of inspection.
13. No dust was observed.
14. The vegetation south of the compost windrow was quite dense during the inspection preventing a thorough
inspection of the area. Please have this area mowed on a quarterly basis as required by Permit Condition 917.
15. An adequate fire lane must be established on the north side of the receiving area for grass/leaves, between the
mulch windrow and the aforementioned receiving area. Any logs located within this area should be moved to the
proper receiving area to allow for access by firefighting equipment.
16. All unprocessed material was being received in those areas designated on the site plan located within the
approved operations plan.

17. Unacceptable material removed from incoming loads bad been placed in the reject area as outlined in the
approved operations plan. Waste consisted of shutters, plywood, and a water hose. A second pile of
nonconforming waste was observed at the edge of the paved areajust west of the chipper. Waste in this area
consisted of construction fence, cardboard, and plastic. Please continue efforts to remove unacceptable waste
streams as they are observed.
18. The facility appeared to be in compliance with all required buffers.
