The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and
any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. The site is a permitted Septage Land Application Site consisting of 2 fields totaling approximately 10. 8
acres (Field `B"- 5.7 acres, Field "C"- 5-1 acres).
2. The site is permitted for land application of domestic septage, portable toilet waste and grease septage.
3. Total maximum application rate is 50,000 gallons per acre per year, and total maximum application rate for
the site is 540,000 gallons per year.
4. The site has an approved above ground septage detention system (30,000 gallon steel tank). The tank has
been installed but is currently not in use.
5. Orchard grass and fescue vegetative cover in place as per the approved Nutrient Management Plan.
6. Good vegetative cover over both fields.
7. The approved Nutrient Management Plan requires the fields to be rotationally grazed or harvested. Field
"B" has had no recent septage applications in preparation for harvesting of hay.
8. Field "C" currently being grazed by cattle as approved in the Nutrient Management Plan. Appropriate
signage in place as per requirements.
9. The site has accessible all-weather roads.
10. Set backs from land application areas to property lines (50 feet) and wells/residences/businesses (500 feet)
are maintained.
11. Disposal area boundaries clearly delineated with permanent markers.
12. Access to the site is controlled by a locked gate.
