OBSERVED VIOLATIONS
None
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and
any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. The Septage Detention and Treatment facility consist of two 10,000 gallon above ground tanks for storage
and treatment of domestic septage and grease trap pumping.
2. Reviewed permit conditions and set-back/buffer requirements.
3. The storage tanks are located at the site of a permitted septage land application site, Permit SLAS-79-02 and
are utilized as a detention/storage facility for septage land application sites SLAS-79-02 and SLAS-79-06.
4. Appropriate signage (No Trespassing signs) is posted for the site.
5. All weather roads are maintained for access to the site.
6. Reviewed pumping and land application logs associated with the site. The tanks are minimally utilized.
7. The tanks at the site appear to be structurally sound and well maintained.
8. The area around the detention and treatment area was found to be free of spills and debris.
9. No odors observed in the area of the detention and treatment area.
10. Approved setbacks are being met.
11. The area around the tanks was observed to be free of vegetative growth as per rule requirements.
12. The septage detention and treatment facility was found to be clean and well maintained.
13. Note: Permit SDTF-79-02 shall expire on October 16, 2014. A permit renewal application for the site
should be submitted by July 16, 2014.
