The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a
Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may be subject
to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service
and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
On September 11, 2013, John Patrone met with Keith Lloyd to conduct a septage management firm inspection of
Lloyds Portable Toilet Rentals & Septic Tank Pumping on NC 87 in Graham, Alamance County.
1. Two septage trucks are listed on the septage management firm permit for Lloyds Portable Toilet Rentals &
Septic Tank Pumping.
2. The septage trucks inspected are license tag numbers: AJ-2700 and AJ-2699.
3. The septage trucks haul domestic septage and portable toilet waste.
4. Septage truck vehicle identification numbers (VIN) were verified.
5. Septage truck tank capacity is as follows: truck VIN ending with 8397 is 1300 gallon and truck VIN ending
with 5194 is 500 gallon.
6. Lloyds Portable Toilet Rentals & Septic Tank Pumping counties of operation are as follows: Alamance,
Caswell, Chatham, Guilford, and Orange.
7. The septage management firm is permitted to dispose of septage at the East Burlington Wastewater Treatment
Plant (WWTP) from Alamance County and the Mason Farm WWTP from Orange County.
8. Mr. Lloyd stated that the authorization to discharge septage at the Mason Farm WWTP encompasses Caswell,
Chatham, and Guilford Counties. It was suggested that Mr. Lloyd ensure that the Mason Farm WWTP
include a statement "and surrounding area" on future authorization to discharge septage forms.
9. The septage trucks contain required signage. Ensure peeling lettering is replaced.
10. Septage truck lettering was > 3 inches.
11. The septage tanks appeared structurally sound and integral to the truck frame.
12. Caps were present on access ports.
13. No visible leaks were noted.
14. Septage management firm pumping logs for both trucks were available for review. Records from January 1,
2013 through September 10, 2013 were verified.
