The Notification is described by the deed recorded in BOOK: 550 PAGE: 093. PURPOSE OF SITE VISIT: Follow-up Compliance Inspection STATUS OF PAST NOTED VIOLATIONS None. OBSERVED VIOLATIONS None. Page 1 of 3 NORTH CAR()LINAD_E Q�� OepaNnent W Envimnmenbl Wel� FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. ADDITIONAL COMMENTS: A copy of the new LCID rules, which became effective on January 1, 2021, are provided with this report. Please read through the rules and contact me if you have any questions. Based on rule 15A NCAC 13B .0563 (6), within 5 years of the effective date, notified LCID landfills will need to be properly closed or obtain a permit from the Division of Waste Management. 1. The grassy vegetation of the notified LCIDLF was needs to be mowed to allow for proper inspection. The eastern side slope was overgrown with woody vegetation. The woody vegetation needs to be removed from the eastern side slope. Several pieces of uncovered concrete and metal were identified at different locations on top of the notified LCIDLF. The metal needs to be removed and properly disposed of at a licensed facility. 2. An area of subsidence and several ruts, which were holding water, were identified in the southern portion of the notified LCIDLF. The area of subsidence need to be repaired and sloped to properly shed water away from the waste 3. A sign is posted on the gate at the entrance showing the contact name and number in case of emergency. 4. The access road is of all-weather construction and appears to be properly maintained. 5. Edge of Waste Markers (EOWMs) had been installed and were identified during this inspection. 6. A large mound of soil comingled with waste that may be from ditch cleaning, was located to the west of the notified LCIDLF. As discussed in the February 2016 and September 2020 inspections, miscellaneous plastic items and aluminum cans were identified within what appears to be ditch cleaning dirt. During this inspection, it appeared that an exterior layer of the soil mound had been scrapped off, therefore, removing a layer of the waste. Continue to remove and properly dispose of the unacceptable wastes from the soil on a frequent basis. 7. Digital photographs were taken during the inspection.
