You are hereby advised that, pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for
each violation of the Solid Waste Statute or Regulations. For the violation(s) noted here, you may be subject to enforcement actions
including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such
further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

AREAS OF CONCERN AND COMMENTS:
1. Site is also approved as a disaster debris site.
2. No un-approved wastes were observed.
3. Several piles of land clearing debris were on site. (Photos #1 & #2)
4. All debris is ground into boiler fuel for use at Evergreen Packaging in Canton, NC.

5. Records of amounts of waste received and the amounts of boiler fuel distributed are maintained at Parker
Lumbers offices at 1960 US Hwy 19E, Burnsville, NC 28714.
6. Access to the site is controlled by a locked gate. (Photo #3)
7. The operations are in compliance with all buffer requirements.
8. A small area of standing water was observed near the east end of the T&P area. (Photo #1) The facility
must be maintained to ensure that there is no standing water.

9. There was no evidence of any flies, rodents, or other insects or vermin.
10. There is no fire control equipment on site. There is no water on site.
11. The West Yancey Volunteer Fire Department (828 - 682-1447) is available for fire control.
