This is an annual notification and requires the annual submittal of a notification form by June 1, 2011. PURPOSE OF AUDIT: Comprehensive Audit of Notified Treatment & Processing (T & P) facility NOTICE OF VIOLATION(S): N/A STATUS OF PAST NOTED VIOLATIONS: The Notice of Violation issued on February 26, 2010 for operation of a T & P without a Permit or Notification has been resolved. AREAS OF CONCERN AND COMMENTS: 1. All photos were taken by Troy Harrison on January 4, 2011. 2. The facility is located on an NC -Department of Transportation (DOT) right -of way and next to an LCID landfill generated from debris from the Hwy 107 construction project. The Notification is active until June 1, 2011. 4. 15A NCAC 13B .0301 requires an operations plan approved and followed by the facility. The operations plan is included with the Annual Notification Form. 5. The hours of operation depend upon demand by the DOT. This facility only takes waste from DOT maintenance operations. Grinder Grinder and mulch piles 6. The grinder is not owned by the DOT and will be brought to the site only when needed. The grinder was not in operation during the audit. DOT staff stated that most of the grinding in Jackson County will be done at the various highway work sites due to cost. 7. The site was not in a flood plain and appeared to meet the minimum buffer requirements.
