AREAS OF CONCERN AND COMMENTS:
1. Directions to site: Exit I-26 to Hwy 25 South toward Zirconia. Exit #3 off Hwy 25, take right on Green River
Road (CR 1106) and travel approximately 6 miles. Facility is on left site of road.
2. Site appeared to meet buffer requirements. A creek runs along the West side of the facility — materials were
—50 feet from the surface water as required. Please ensure that materials are not placed within this buffer zone.
Additionally, materials should be maintained at a distance of 50-feet to all property lines, including Green
River Road.
No unacceptable waste was noted in the mulch piles on site. Material was placed in windrows and appeared to
have adequate access (fire control). No standing water was noted on site. Less than 6000 yds3 present at time
of inspection.
4. Please continue to maintain operational requirements as outlined in your Notification form.
