The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and
any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. On site to conduct inspection of the Treatment & Processing Notified facility. Site was previously operated
under TPN-11-026 Monticello Crossroads. The August 9, 2011 inspection stated that the Operator's lease of
the property had not been renewed and that the Notification to the Section had not been renewed. New
property owners (Riverside Stump Dump) have taken control of abandoned materials and established an
active grinding operation.
2. Facility had signage posted at entrance, roads were accessible, operator was present (SRay).
3. It appeared as though the abandoned land clearing wastes noted during the previous site inspection had been
ground and sold/removed. Newer wastes and mulch piles were observed on site. All materials met
Notification requirements and the material was <6000 cubic yards.
4. No standing water was observed and it appeared as though buffer requirements had been maintained.
5. Please note that the collected concrete (from cleanup of the property) contained some wastes considered
unacceptable for beneficial fill use (primarily metals, rebar). If the facility intends to use this material, ground
or otherwise, for land improvement activities, please ensure that the material contains only: clean concrete,
brick, block, rock, and uncontaminated soils and that the use does not involve excavation or create erosion or
surface water impacts.
