STATUS OF PAST NOTED VIOLATIONS:
Additional logs that were submitted are currently under review. However no site logs have been received since January
– submit additional logs at this time (February 2021 – current).

OBSERVED VIOLATIONS:
.0842(a)(17) & Permit Condition 2. Approved nutrient management plans shall be followed. This site shall be operated
and maintained in accordance with the submitted nutrient management plan. As per the plan the rye cover crop shall be
cut in March or April. Currently the rye is very tall on Field 2 and needs cut asap in accordance with the 30 day
wait after the last septage application.

with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

.0842(a)(12) All septage discharges, including aerial drift from discharges, shall be made within the permitted
boundaries of the land application site. Septage waste was noted on the ground surface within the 500 radius
setback on field 2 (at more than one location). SEE BELOW

All septage waste shall be removed from within the 500 foot radius setback on Field 2 and measures implemented
to prevent this from occurring in the future.

Excess vegetation needs removed from around the posted sign on the entrance road and from around various
field markers on Field 2.

ADDITIONAL COMMENTS
1. Implement weed control as needed particularly on Field 3 (the new field). Continue efforts to maintain the
required cropstand on Field 3 – noted sparse areas of cropstand at this time on this field. Field 1 has been
harvested and the cropstand appears adequate on this field.
2. Markers appear adequate on Fields 1 & 3 – hard to see markers on Field 2 at this time due to tall grass.
3. No apparent sign of heavy or recent application. No odors noted.
4. The required signs are posted and the access road is adequate.
5. The Raleigh Central office received a copy of the recent soil analysis.
