The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. The two currently permitted tanks appear structurally sound. It appears that septage is being transferred to/from
the facility in a safe and sanitary manner. No spills or leaks noted. No odors or vectors noted. The approved
setbacks are being met. The required 5 foot cleared radius (free of excess vegetation or debris) is present around
these two tanks at this time. The required signs are posted (slightly obscured by vegetation) and the access road
is adequate.
2. Additional tanks & a roll off box has been located near the new area to be modified for a treatment facility. The
tanks & box are empty at this time and are not currently permitted for use. The 200 foot setback (double setback
for a high capacity facility) from the creek shall be maintained for this treatment facility. The following items
shall be submitted for the permit modification – SDTF application, site drawings, operation manual, leak test
(signed by a PE) for any buried tanks or lines and a landfill disposal authorization for the screening debris.
Treatment logs will be required when the permit modification is issued and authorization forms for any firm
other than Belton’s Septic Tank Service that may utilize the facility.
