OBSERVED VIOLATIONS:
1. .0842(a)(12) All Septage discharges, including aerial drift from discharges, shall be made within the permitted
boundaries of the land application site. Septage waste was noted on the ground surface in various locations within
the 500 foot radius setback on Field 3. Septage was also noted outside the permitted boundary on Field 3 on the
adjacent perimeter road. Measures will be implemented to prevent any septage discharge (including any
aerial drift from the discharges) from landing on the ground surface outside the permitted boundaries on
this site. No septage shall be within the 500 foot setback or on the perimeter road adjacent to Field 3.

with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Field 3 was GPS’d to create an accurate site map. Measures should be taken to control the understory growth
on this field. Noted areas of heavy application – review discharge procedures. No odors noted at this time.
Field markers appear accurate on this field but setbacks were not re-measured during this visit. Field 3 is the
only disposal field approved to be in use at this time.
2. Significant work has been done to clear understory growth on Field 2 and the situation is greatly improved. All
field markers will have to be accurately posted for land application to begin again on Field 2 in August (in
regards to previously exceeding application rates). Field 1 is currently inactive due to past timbering.
3. The required signs are posted & the access road is adequate.
4. Additional tree plantings should be done on all of the fields (under the guidance of Forestry) to fill in gap areas
among the tree spacing in various locations.
