PURPOSE OF SITE VISIT: Routine Inspection
STATUS OF PAST NOTED VIOLATIONS: N/A OBSERVED VIOLATIONS: 15A NCAC 13B .0842 (a)(7) states: “septage shall be applied to the surface of the land from a moving vehicle to have no standing liquid or soil disturbance resulting from the waste flow after the discharge is complete” (see photos in body of this report). Myers Septic Tank, Co. was in violation of because Field 1 because a recent land application was observed to have
resulted in soil disturbance. In order to come into compliance, Myers Septic Tank, Co. must repair the damaged area and submit photos confirming
the repairs by May 14, 2021.

ADDITIONAL COMMENTS
1. The Septage Land Application Site consists of Field 1 (2.0 acres) and Field 2 (4.4 acres) for a total of 6.4 usable acres. 2. The site is permitted for land application of domestic septage. 3. The site had accessible all-weather roads to the septage land application field covered under the permit.
4. Proper signage was located at the entrance to the fields.
5. Both fields had disposal area boundaries that were delineated with permanent markers; however, markers on the Northeastern corner of Field 1 and between fields 1 and 2 were missing. These need to be replaced. 6. All septage discharges appear to have been made within the disposal area boundaries.
7. A recent land application was observed on Field 1 which resulted in damaged crop and soil disturbance in violation of 15A NCAC 13B .842(7).
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

8. No objectionable odors were observed. 9. The nutrient management plan has fescue and Bermuda grass as the established crop. Areas that develop less than 80% groundcover by fescue must be reseeded with tall fescue.
10. Both fields appeared to have recent land applications. As a reminder, Myers Septic Tank Co. must follow the Nutrient Management Plan (NMP) as prescribed specifically to these fields. The requirements are listed below
from the NMP.
