OBSERVED VIOLATIONS: While none are noted in this report, the damage to the liner system noted in the comments below remains under cooperative investigation between the Section, the County, and its consultants. Any violations of law or rule found as result of the investigation could result in tiered enforcement action.
ADDITIONAL COMMENTS On February 16, 2021 Chuck Kirchner conducted a partial inspection in response to a report that facility staff discovered damage to the base liner system located on the northwest side in Phase 3 between Cell 1A & 2A. On February 24, 2021, a follow up inspection was conducted with Permitting Branch staff and Field Operations Branch staff.
1. Plan sheet section shown below from December 6, 2018 LFG Monitoring Report Plan Sheet was used for area identification in this report.
North is to the left on the plan sheet above.
2. In the summer of 2020, the facility began placing waste in an area in Phase 3, Cells 1A and 2A which had been under a rain cover since the construction was completed in 2006. The facility first started removing the rain cover and placing waste in the southwestern corner of in Cell 1A. The current working face is located in the
eastern corner of Cell 2A. 3. The base liner system in Cell 2A on the north side, along the access road, was discovered as damaged as the facility continued to remove the rain cover to place waste. It appeared that the GCL layer had separated at the
seams. Soil cover was not visible on the upper slope over the GCL layer.
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

4. The integrity of the base liner system and the possible release of leachate is under investigation. Continue to provide complete and timely responses to the Permitting Branch staff request for information as it pertains to the investigation of cause and creation of a repair work plan.
5. The MSW working face was small and compact. The photograph below is of the active working face. The bottom of the photograph shows the base liner with tears in the seams at the left and right.

6. During the 2/16/2021 inspection, Spray-applied Mortar Coating (SMC) was being sprayed on compacted waste to the west of the working face. Some waste was not completely covered. SMC shall completely cover all exposed waste surfaces so that only the coating is observable.

7. Soil cover is required to be applied within a 7-day period. No soil cover was observed in the basin area during the inspections. Ensure 6” soil cover is applied at least once weekly.
8. All liquids above and below the rain cover, as shown in the photograph above, are being treated as leachate and being pumped into the leachate collection system. 9. No windblown material was observed. 10. Repair eroded rills on landfill cap and continue to seed and mulch until cap is stabilized with vigorous permanent grass cover. No exposed MSW observed in eroded rills.
11. There appears to be some discrepancy between historic drawings of the landfill in Phase and Cell identification numbers; please provide updated information to the Solid Waste Section Permitting Branch.
