STATUS OF PERMIT:
Active. PTO Issued February 18, 2021 and expires September 17, 2064. PURPOSE OF SITE VISIT: Comprehensive Facility Compliance Inspection. STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS:
None
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS:
01 - This facility has required signage at the entrance and directional signs inside of the landfill footprint.
02 - Roads are constructed of all-weather materials and are being maintained well. Facility has locking gate and fencing for security. 03 - Edge of waste markers present with exception of a few that the mower has hit. Need to replace the missing markers. 04 - Monitoring wells are locked and tagged.
05 - Working face of this facility is kept small, and cover material is stored on the working face for quick access. Non-conforming waste is being removed from any load and taken to the proper site for disposal. 06 - Facility has adequate soil cover pile on-site for use.
07 - Land Clearing and Inert Debris area is well maintained and is being covered monthly at a minimum. Site is less than 2 acres. 08 - Asbestos area is marked and is being managed well. 09 - Screening records were reviewed and are up to date and has required information. 10 - There is a Wood Processing and Yard Waste Composting Area located in the landfill footprint. This site is managed well. Temperature records are being kept for Pathogen reduction and Vector attraction reduction.
11 - The wood processing area has adequate fire lanes in the event of a fire. Very little material on-site today. 12 - All White Goods and scrap metal are stored in open top containers. White Goods containing CFC’s are placed into
the open top containers upright for Foss Recycling to remove the CFS at their facility. A yearly record is provided to this facility from Foss Recycling on the CFC removal.
13 - There is a Recycle/Electronics Center located in the landfill footprint. This center is managed well.
14 -There is a Convenience site located inside of the landfill footprint. Containers are labeled with the type waste
received. This site is managed very well. 15 - All MSW waste from this Convenience Center goes to Waste Industries, Black Creek Road Transfer Station, permit # 9808T in Wilson, NC.
16 - It was noted that a small amount of windblown trash will need to be picked up by the end of the working day. 17 - Ground water well testing reports were reviewed during this comprehensive facility compliance inspection. Reports
reviewed were dated; 5/21/2020 and 11/5/2020. 18 - Surface Water sampling reports reviewed and dated; 5/21/2020 and 11/5/2020. 19 - CFC removal records were reviewed. 20 - Landfill gas monitoring reports were reviewed and dated; 3/13/2020, 6/12/2020,9/14/2020 and 12/14/2020. 21 - Incoming load tickets were reviewed at the scale house and were up to date and have required information. 22 - Permit to operate for this facility was issued 2/18/2021 and expires 9/17/2064. 23 - Facility Certified Operators; Andrew Davis, MOLO – exp.8/6/2023, Paige Aycock, exp.10/19/2022, Kenny Matthews, exp. 10/19/2022, Melinda Wall, exp. 10/19/2022, Tammy Webb, exp. 11/15/2022, Robert Patterson, exp.4/11/2023 and J. Mark Denton, exp. 6/13/2022. 24 - Digital pictures were taken during this comprehensive facility compliance inspection. Some are attached to this report.
25 - Overall, this facility is being managed well.
