Mr. Holsomback,
Hello. Hope you are doing well. I am sending you this email to let you know that the Permit, SDTF‐69‐
03, covering the below grade 1,200‐gallon concrete tank for the storage of domestic septage discharged
from recreational vehicles facility expired onMarch 30, 2020. This permit is different from the Permit to
Operate a Septage Management Facility, Permit # NCS‐01262, issued on March 2, 2020. A notification
for permit renewal should have gone out 4 months before the permit expired however, one was never
sent to you.
I have attached a notification letter that covers the information that needs to be submitted, an
application for permit renewal, landowner authorization just in case you are not the sole owner of the
property that the tank is own, and a copy of the previous permit. The previous permit also has the
application that was last submitted. This may help you fill out the application. Please get this
information to me as soon as possible. If you have any question, please contact me.
Thanks,
Chester
