is hereby issued a Septage Management Firm Permit, Permit Number NCS-01565
And by virtue of completing the annual training
requirements is hereby registered as a Portable Sanitation &
Septage Management Firm
in the State of North Carolina.
This permit to operate a Septage Management Firm is issued to the above named person, business or entity alone and is not transferable to any other person, business or entity.
Firm operation shall be in accordance with the provisions of N.C. General Statute 130A-291.1 - 130A-291.3, Title 15A of the N.C. Administrative Code 13B .0800 et.seq., conditions of the permit, and representations made in the application and accompanying documents for a permit. The permit holder is authorized to discharge septage only at the locations(s) listed below: 1. Town of Siler City WWTP
This permit does not entitle the permit holder to operate a Septage Land Application Site, a Septage Detention or Treatment Facility, or any other solid waste management facility not specified herein.
Failure to operate as permitted may result in the Department suspending or revoking this permit, initiating action to enjoin the unpermitted operation, imposing administrative penalties, or invoking any other remedy as provided in Chapter 130A, Article 1, part 2 of the North Carolina General Statutes. This permit and registration expires on December 31, 2021.

Dear Mr. Cheek:
We have received an Application for a Permit to Operate a Septage Management Firm, a permit fee
payment in the amount of $1,100 (double fee charged for operating without a permit as per N.C.G.S.
130A-291.1(c)), and a disposal authorization for the Town of Siler City WWTP. When the Permit to
Operate a Septage Management Firm is issued, the Firm Permit Number will be NCS-01565.
Section .0844 of the Septage Management Rules provides the requirements for lettering your vehicle(s).
However, your Application to Operate a Septage Management Firm will not be processed until you
have attended a New Operator Class offered by the Division and the vehicle(s) has been
inspected for compliance with the rules and approved by the Division. The required vehicle
lettering must be no less than 3 inches in height and must show the firm name, town name, phone
number, and permit number, NCS-01565. This lettering must be shown on both sides of your vehicle(s)
and must be permanently attached to the vehicle(s). When the required lettering has been completed,
please contact Ms. Connie Wylie, Environmental Program Consultant, in the Fayetteville Regional Office
at 910-433-3352 or email at Connie.Wylie@ncdenr.gov to request a truck inspection.
This letter shall not be considered as a permit to operate a Septage Management Firm. Please note
that you may not legally operate a septage management firm in North Carolina without a permit.
General Statutes, GS 130A-291.1 (c) states in part “A septage management firm that commences
operation without first having obtained a permit shall cease to operate until the firm obtains a permit
under this section.” You are hereby advised that, pursuant to N.C.G.S. 130A-22, an administrative
penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Statute or

Mr. Cheek,
Good morning. It was a pleasure meeting you yesterday. As discussed, I am sending this email with
instructions for obtaining a Septage Firm Permit and to avoid an administrative compliance order. I will try to
be as thorough as possible, but please feel free to contact me, Jeff, or Connie with any questions or guidance
you have regarding the below information.
1. Please submit a payment within thirty (30) days in the amount of $1,100 ( annual firm permit fee of
$550 plus illegal pumper fee of $550 in lieu of a compliance order). Please make the check payable to
the “Division of Waste Management” and send the check to the below address:

2. Please contact Jeff Bullard (copied on this email) to register for the required New Operator’s Septage
Firm Class. His contact number is 919-707-8285. This class will be conducted on Wednesday, March
24, 2021. This class is Required . The class is free and will be conducted virtually ( by computer
and/or phone).
3. Complete and submit a Firm Permit Application. I have attached a copy of one along with the required
Waste Water Treatment Plant Authorization Forms.

4. Once processed, we will issue you an NCS number. At this time you can property label your
truck. Trucks(s) will need the lettering on both sides of the truck(s); the lettering height must be at least
three inches on ALL letters/numbers. The following must be on the truck(s): the company name as it
appears on the firm application, telephone number, town/city you will operate out of, and the
NCS permit number.
5. Once your truck is labeled, please contact Connie Wylie (copied on this email) to setup a truck
inspection.
Once the above steps are completed you will receive a septage firm permit for this year. I am including some
websites below with all this information along with required forms and rules. Most of this information you will
learn in the required training class but it is useful to have this links handy.

It is important to know that you currently do not have a valid permit to pump septage in North Carolina. Please
discontinue pumping septage until you have received as valid permit. Failure to do so will result in additional
