The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. The required signs are posted at the site entrance. The access road is adequate.
2. The field posts appear accurate. It appears that the approved setbacks are being met but were not measured at
this time. No apparent sign of land application outside the permitted boundaries. No sign of heavy or recent
application. No odors noted.
3. Noted various fields that appear to have an adequate cropstand at this time and other fields or parts of fields that
appear discd up. Field 15 does not currently have a cropstand and Field 8 has tall weeds on this field – issues
were noted with both of these fields on the last site visit. The required cropstand needs to be sufficiently
established on all fields. Weed control is needed in the coming warm weather months. The last audit report of
10/22/2020 noted that the local extension agent needed to be contacted for advice on weed control &
reestablishing the cropstands as needed – if this advice was not obtained please do so now.
4. Soil samples were taken last year and the Raleigh Central office did receive a copy of the soil report.
