The corrective actions listed in the Notice of Violation issued on February 23, 2021 have been completed by March 25,
2021. The Notice of Violation has been resolved at this time.

with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspected vehicle tag#YA145460, VIN correct per the firm application, capacity 2500 gallons.
The tank appears structurally sound & securely attached. Outlet covers were on and no drips/leaks were noted.
The lettering is correct on both sides of this vehicle per the information on the firm application. This vehicle is
in compliance at this time.
2. The Raleigh office has already received the required fee, application form & one disposal authorization. Mr.
Cheek attended the new firm operator class on March 24, 2021. The firm permit was issued on March 25, 2021
at which time septage pump jobs could begin.
3. Discussed adding more disposal authorizations in the future – for other areas & to handle grease septage.
Contact this office if in the future you wish to add another vehicle to this firm permit. A permit is required prior
to the use of any storage tank to hold septage or to land apply septage – contact this office for info as needed.
