PURPOSE OF SITE VISIT: Pre -operation Inspection. STATUS OF PAST VIOLATIONS None. OBSERVED VIOLATIONS None. Page 1 of 4 NORTH CAR()LINAD_E Q�� OepaNnent of Envieonmenbl W.I� FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. ADDITIONAL COMMENTS: The purpose of this inspection was to complete the required pre -operation inspection, prior to accepting waste at the newly constructed LCID. Upon arrival, the Section was informed that the facility had received approximately 1.5 inches of rain that morning. 1. The sediment basin to the northeast of Phase I of the LCID has been constructed. Three porous fabric baffles run the length of the sediment basin. The sediment basin also consists of a skimmer, concrete riser, and an emergency spillway. The side slopes of the sediment basin have been seeded and lined with fabric to help prevent erosion and facilitate the growth of permanent vegetation. 2. Riprap has been placed on the downstream side of the outfall. During this inspection, the water exiting the outfall was extremely turbid and flowing into the forested area adjacent to Pig Basket Creek. Mr. Fortner stated that the water was still very turbid after running through a filter. The turbidity of the outfall needs to be monitored, so that the excess sediment does not inundate the forested area. DEMLR staff have been notified and will visit site. 3. The drainage ditch along the southern and eastern edges of Phase I of the LCID has been constructed. The drainage ditch has been seeded and lined with fabric to facilitate the establishment of permanent vegetation. The grasses are expected to ensure erosion control. Any repairs to the ditch and/or established vegetation need to be completed immediately. 4. One area of riprap was identified within the ditch, at the sediment basin inlet pipe. Monitor the drainage ditch to determine if additional riprap areas are needed to prevent erosion in the drainage ditch. 5. Vegetation was still located within the Phase I LCID cell. Mr. Nichting and Mr. Fortner stated that since there was no excavation required for the LCID, the vegetation would be scraped off and the cell would be ready to receive waste. If there is no issue here with separation from the seasonal water table, it may be possible to remove any trees and bush hog the remaining area to assist in stabilization of this area. 6. The roadway to the Phase I LCID cell was not complete at the time of this inspection. Mr. Fortner stated that the roadway would continue to the southern tree line and terminate in a cul-de-sac. This would allow for vehicles to turn around and dump their LCID waste. Another option is to continue the roadway around the entire Phase I and II LCID cells. This would help with the flow of traffic. 7. Once the LCID is operational, an operator shall be on duty at all times while the LCIDLF is open for public use to direct patrons, monitor and remove unacceptable waste, and compact/cover the waste (15A NCAC 13B .0566 (6)). 8. The site must be completed before operations begin. Please notify Section Staff for follow up site visit. 9. Digital photographs were taken during the inspection.
