FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 ADDITIONAL COMMENTS 1. Inspected the permitted vehicle, tag# & VIN# correct per the firm application, capacity 4000 gallons. 2. The tank appears structurally sound & securely attached. 3. Outlet covers were on and no drips/leaks were noted. 4. The lettering is correct on both sides of this vehicle -however the phone number shown on the truck is the phone number listed on the firm application in 2017, not the phone number that has been listed on recent applications – this should be corrected on future applications. 5. Reviewed the truck logs - discussed septage pump jobs, using the storage tank, disposal locations etc. It appears that the required information is being maintained. Also discussed the truck tank cleanout procedures which appears to be in compliance. Discussed attending a continuing education class – contact Adam Ulishney (919- 707-8210) if you are interested in the online class in April. Thank you for your time.
