The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Reviewed the truck pumping logs. Discussed the required information to be maintained – it appears that the
needed info is being recorded.
2. Inspected the two vehicles covered under this septage management firm permit.
Tag#YA109034, VIN# correct per the firm application, capacity 2500 gallons. Outlet covers were on and no
drips/leaks were noted. The tank appears structurally sound & securely attached. The lettering is correct on both
sides of this vehicle per the information on the firm application. However the permit number is very faint
and needs to be repainted on both sides of this vehicle asap.
Tag#YA155173, VIN# correct per the firm application, capacity 3100 gallons. Outlet covers were on and no
drips/leaks were noted. The tank appears structurally sound & securely attached. The lettering is correct on both
sides of this vehicle per the information on the firm application. However it appears that ‘possibly” the town
name may be less than three inches on both sides of this vehicle/measuring on a curve – but this vehicle was
found to be in compliance in 2012 by this inspector.
3. Mr. Hardee stated that he was registered to attend a continuing education class this spring.
