The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspected two additional vehicles to be added to the firm permit.
Tag#MX6642, VIN#1NPTL40X4ED218850, 3400 gallon capacity.
Tag#PV45405, VIN#10BGS6217SF0A5175, 6000 gallon capacity.
Outlet covers were on both vehicles and no drips/leaks were noted. The tanks on both vehicles appeared
structurally sound & securely attached. The lettering is correct on both sides of both vehicles per the
information on the firm application – except on vehicle tag#PV45405 the “&” symbol is missing between the
“A” and “D” in the firm name. Discussed ways that this could be corrected in the space available for the
lettering.
2. Reviewed the truck pumping logs – it appears that the required information is being maintained at this time.
3. Four vehicles (two on this date and two on 2/17/2021) have been inspected and may be used for septage pump
jobs under this firm permit. Contact this office in the future if additional vehicles are needed – an inspection
shall be required.
