The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
ADDITIONAL COMMENTS:
1. The facility is a closed, unlined sanitary landfill that entered the post -closure period on May 6, 1991. A partial
history of the site is as follows:

o Sanitary landfill operations began in 1976, and the facility accepted waste until 1988.
o On 10/5/1989 and 12/7/1989, Solid Waste Section (SWS) staff determined that the sanitary landfill had
been properly closed, with the exception of a 2.07-acre area that had been used as a demolition landfill.
o On 1/2/1990, the SWS issued a letter approving the continued operation of a demolition landfill on the site
until such time as the final grades for the landfill were reached.
o On 2/28/1991, proper final cover of the demolition landfill area was verified by the SWS.
o On 5/6/1991, a closure letter was issued for the entire sanitary landfill site.
o On 12/7/1992, an approval letter was issued by the SWS to the City of Winston-Salem for the operation of a
new, permitted demolition landfill (Permit #34-90-07) at the site, until such time as final grades for the
landfill were achieved.
o On 5/25/1994, an LCID notification was recorded in the Forsyth County Register of Deeds (Book 1825,
Page 1095, 1096) for an LCID landfill contiguous to the closed sanitary and demolition landfill units at the
site.
o On 3/2/2010, a notice of closure for the notified LCID landfill was recorded in the Forsyth County Register
of Deeds (Book RE 2935, Page 270).
2. Photos included in this report were taken by Susan Heim on February 23, 2021 during the inspection.
3. No environmental monitoring takes place at the facility. However, monitoring ports had been installed on several of
the landfill's passive gas vents at the time an office trailer was placed at the impoundment lot. The solid waste staff
does not monitor the ports or receive reports of any monitoring activity.
4. The facility is secured by a perimeter chain link fence, and unauthorized entry is prevented by means of two locking
gates. A portion of the facility is paved and used as an impoundment lot for abandoned vehicles by the City of
Winston-Salem. The impoundment lot is secured by a separate perimeter chain link fence and locking gate.
5. Permanent edge -of -waste markers indicate the approximate limits of the landfill footprint and are visible around the
perimeter of the disposal area. The markers were installed in 2009 after extensive hand auguring of the site.
6. The facility is accessed from Old Milwaukee Lane. A small gravel area is located inside the western gate, and an
old access roadway extends from the gravel area around to the perimeter of the northern slope of the landfill.
However, the access roadway is not maintained, and the site is accessible only on foot.
7. Litter and windblown trash were observed along the facility's Old Milwaukee Lane frontage, both inside and
outside the perimeter fence. Please ensure that all litter is collected and removed for proper disposal.
8. The top of the landfill is covered by a thick vegetative cap. No ponding water was observed on the landfill top.
9. Stands of trees, most 6" or less in diameter, have become established on the landfill's northern slope as well as
along the edge of the disposal area along Old Milwaukee Lane. The northern slope is free of undergrowth and fallen
limbs, and appears to be stable, without evidence of erosion.
10. The eastern slope is mostly covered with mature trees. Because of the wet conditions and the steepness of the slope,
this area was observed only from the top of the landfill. Areas of the eastern slope appeared to have been eroded
over time, but no uncovered waste was noted.
11. The southern end of the landfill, which abuts the city's impoundment lot, is wooded. Because of the wet conditions
and thick vegetation, this area was observed only from the impoundment lot and from Old Milwaukee Lane. No
uncovered waste was visible.
12. Please ensure that the landfill is inspected regularly and repaired as necessary to protect the integrity and
effectiveness of the cap.
